NOTE:  This form is strictly to withdraw and transfer events.  If you wish to cancel an event and receive a refund please login to your account with your id and password.  Once you have logged in, go to the tournament tab - registration and click the "cancel" link next to the event you wish to withdraw from and follow the directions as provided.  Thank you.  To see the refund policy click here.

You may not transfer to a Fall Series Event until July 26, 2008.  Any transfers received will be discarded.

 

Once you submit your request, you will be immediately notified that your withdraw and transfer has been sent to GOLF.  Your request will be complete when you receive a confirmation email from GOLF.
